Thomas “Tom” Slezak, 75, passed away on Tuesday, January 22, 2019 with his wife by his side. A celebration of life will be held at 2 p.m. Wednesday, January 30, 2019 at Southcliff Church, 4100 SW Loop 820, Fort Worth, Texas 76109.

Tom was born on July 14, 1943 in Cicero, Illinois to Steve and Elizabeth Slezak. He loved his Lord. He enjoyed making crafts, doing things with his hands, shopping with his wife and going to the beach. Tom served as a police officer for the Muskegon Township Police Department in Michigan. He loved being around people, especially his family.

He is survived by his wife Patricia Slezak; daughter: Rhonda Smith and husband Robert; sons: Thomas Slezak Jr., Steven Slezak and wife Jamie; step daughters: Katie Haggard and husband Jared, Lara Hoefelmann and husband Steven; ten grandsons; six granddaughters; two great grandsons; one great granddaughter; brother: Robert Slezak and wife Marilyn; numerous nieces, nephews and cousins.

He was preceded in death by his parents Steve and Elizabeth Slezak, and his first wife Judith Slezak.
